Medical Collections in Illinois: What Can and Cannot Appear on Your Credit Report

The Three Rules That Changed Everything

  • Paid medical collections cannot appear. Once you pay a medical collection, the bureaus remove it entirely — it no longer stays for seven years.
  • Medical collections under $500 are not reported by the three national bureaus at all.
  • Unpaid medical bills get a one-year waiting period before they can appear, giving insurance disputes and billing corrections time to resolve.

Check your reports against these three rules. If a paid medical collection, a sub-$500 medical collection, or a bill sent to collections inside the one-year window appears, it is disputable as a reporting-rule violation.

Why This Matters in Chicago Specifically

Cook County has one of the largest concentrations of hospital systems in the country, and medical billing errors — charges sent to collections while insurance was still processing, financial-assistance-eligible bills billed at full rate, duplicate charges — are a persistent source of wrongful collection entries. Illinois also operates hospital financial assistance requirements under the Illinois Fair Patient Billing Act, and a bill that should have been covered by assistance is worth challenging both with the hospital and on the report.

What To Do With a Medical Collection

  • Verify it against the three rules above — many entries are simply no longer allowed
  • Request an itemized bill from the provider and match it against your insurance explanation of benefits
  • If the bill was insurance’s responsibility or assistance-eligible, dispute with documentation
  • If accurate and over $500: paying it now removes it entirely under the paid-collection rule — one of the few cases where paying a collection directly helps your report
